By filling out New Patient Forms ahead of time you will save significant time on your visit. If you have any questions when filling out the form, please do the best you can, and our staff will assist you with your questions on the day of your appointment.
You can download the following form on your computer and print it to fill out before your appointment, please bring this with you to your first visit.
At your first visit, we will review your medical and dental history. Please bring a list of any medications you are currently taking. As well, please bring any current x-rays or have them forwarded from your previous dentist to our office prior to your first appointment. Our helpful staff can assist you to have your x-rays sent to our office.
Following a review of your medical and dental history we will listen to your concerns. Next we will examine you and perform any necessary tests, x-rays and gather any records necessary for a thorough diagnosis. Based on your concerns and our findings treatment options will be reviewed. With your informed decision, an individual treatment plan will be customized for you.
We encourage patient to give us insurance ahead of time (prior to appointment).
We do offer direct billing if your insurance company allows it. Please bring your insurance card to your 1st appointment. Claims will be submitted electronically or by mail on your behalf. However, we will collect your portion not covered by insurance on the day of treatment. If the insurance company does not give us immediate feedback on the amount that is covered, we will estimate your portion as a percentage of the total treatment. Please be aware that you are responsible for paying all fees not covered by your insurance company. If you need treatment estimates or have any questions, we will be glad to assist you. It is your responsibility to be familiar with what your insurance plan covers including frequencies of procedures, maximums and renewal dates. Our helpful staff can provide you a list of questions to ask your insurance company.
Payment is expected at the time of treatment. For your convenience, we accept cash, debit card, VISA and MasterCard. No personal cheques.
Call our office at 780-486-0788 to schedule an appointment or make an online request. Appointment times are reserved specifically for you in order for us to provide the attentive care you deserve. If you are unable to keep an appointment, please give us 48 hours notice so that we may offer that time to another patient.
Walk-ins may be accepted but it is preferred if you can call ahead of time to prevent or minimize waiting time.
If you are suffering from a toothache or have any other urgent dental concern, we will make every effort to see you that same day. The dentist will examine you and prescribe any necessary medications to provide relief of your symptoms. If there is adequate time, further treatment may be performed or initiated as needed.
